Find A Provider

Use this tool to search for a UnitedHealthcare Community Plan provider or specialist. You can find primary care providers (PCP), specialists, hospitals, laboratories, pharmacies, medical equipment supplies and more.

With our plan, you have a personal doctor to work with for all your FREE health care needs. Your personal doctor is your Primary Care Provider (PCP). You get most of your care from your PCP. Your PCP will give you the care you need or send you to a UnitedHealthcare Community Plan provider or specialist.

When you see your PCP or specialist, tell them you are a UnitedHealthcare Community Plan member. Show them your ID card too.

Drug coverage will have changes starting October 1, 2020. This change is because of a Michigan Department of Health and Human Services policy change.

If you filled a drug that will no longer be covered, you will get a letter. Your doctor may need to switch you to a different drug or request that you to keep taking drugs that will no longer be covered.

You will be able get the same drug until September 30, 2020.

What Should You Do? If you get a letter, please talk to your doctor about the changes in the letter as soon as possible. The letter may provide other drugs to choose. If your doctor feels that you cannot use another drug, they may request a prior authorization for the drug you are taking. It is important that you follow these steps so you will not have issues getting drugs after October 1, 2020.

You must choose a dental provider in the UnitedHealthcare network. The  Provider Directory lists all participating dentists in the network and can be viewed on myuhc.com. Please contact Member Services at 1-800-903-5253  if you have any trouble making an appointment or help finding a network dentist.  

The Healthy Kids Dental program is offered statewide to those who get Medicaid and are ages 0 through 20 years. You will be enrolled automatically. The two plans available are Blue Cross Blue Shield of Michigan and Delta Dental of Michigan. You will get an identification card from the dental plan. It will have the phone number for your plan. You can call your dental plan for help in finding a dentist.
