Medicaid plans

UnitedHealthcare Community Plan - MO HealthNet Managed Care

UnitedHealthcare Community Plan is a MO HealthNet Managed Care plan. Our health plan is for children, families, pregnant women and adults who meet MO HealthNet income requirements. We also offer support to adults with special health care needs. Members can choose their own doctor.

Dual Special Needs plans

Dual Special Needs plans (also called Dual Health plans or D-SNPs for short) are for people who have both Medicaid and Medicare. Dual plans cover most doctor visits, hospital stays and prescription drugs. They offer many extra benefits beyond those you get with either Original Medicare or Medicaid.

Individual & Family ACA plans

UnitedHealthcare offers Individual & Family ACA plans in Missouri. They’re designed to give you the coverage you need without breaking the bank.

Choose from plans in the Bronze, Silver and Gold metal levels. They each come with a variety of benefits, including reliable support from our customer service experts.

MO HealthNet coverage has expanded. Eligibility in the state of Missouri expanded in October 2021 allowing more adults between the ages of 19-64 to qualify based on income.

Our UnitedHealthcare team recently partnered with Dream Builders and the St. Louis Cardinals to distribute 250 coats to the Hickey Elementary students in St. Louis.

MO HealthNet Coverage has Expanded

Eligibility in the state of Missouri expanded in October 2021 allowing more adults between the ages of 19-64 to qualify based on income. If you make less that $17,130 per year, you may be able to receive coverage. If you qualify, you'll receive all the standard MO HealthNet benefits including doctor visits, vision, and dental. Enrollment is through the state. You can see if you qualify, and apply for coverage at mydss.mo.gov

Dual Special Needs plans in 2024

Missourians on a UnitedHealthcare Dual Special Needs plan (or dual health plan) get many extra benefits beyond those you get with either Original Medicare or MO HealthNet.

That includes:

  • A monthly credit to buy healthy food and OTC products or pay utility bills
  • An allowance for covered types of preventive and comprehensive dental
  • $0 copay on yearly routine eye exam and an eyewear allowance with free lenses

MO HealthNet Managed Care care management program

If you’re a UnitedHealthcare member with serious, ongoing health care needs, a care manager can really help.

Your care manager will:

  • Work with you, your family members and friends on a plan of care
  • Explain medical terms in plain language
  • Coordinate your doctor appointments
  • Provide your care team with your medical records
  • Connect you with local resources and services

Speak to a registered nurse anytime 24/7 to get answers to any health-related question or concern.

Our nurses will:

  • Listen to symptoms
  • Help with self-care
  • Advise you about getting care at a doctor visit or an urgent care center
  • Help you know when to go to the emergency room

Transportation

Rides to and from medical appointments are covered if you qualify and have no other way to get there. The appointment must be with a UnitedHealthcare Community Plan network provider.
