UnitedHealthcare Community Plan will not be a Minnesota Health Care Programs health plan option in 2025
We regret to inform you that UnitedHealthcare Community Plan will not be a Minnesota Health Care Programs health plan option in 2025. You will need to choose a new health plan. You should have received an Annual Health Plan Selection (AHPS) notice with instructions on how to select a new plan.
To choose a new health plan, you must return the completed notice by December 6, 2024. You can also choose your health plan by calling the Minnesota Department of Human Services (DHS) Health Care Consumer Support by December 06, 2024, at 651-431-3722 or 833-970-0047 (this call is free). If you do not select a new health plan, a health plan will be chosen for you. You will receive written confirmation of your new health plan in the mail.
At this time, our priority is to continue to serve our enrollees. For the remainder of 2024, you can continue to utilize your UnitedHealthcare Community Plan benefits and see your providers as normal. If you have any questions, please call Member Services at 1-888-269-5410, TTY 711, 8 a.m. – 6. p.m. CT, Monday – Friday.

Our Families and Children Medical Assistance (Medicaid) plan - also known as the Prepaid Medicaid Assistance Program (PMAP) - is for children, pregnant women, families and adults without children. You can choose your own doctor. And you get many extras that other plans don’t offer.
Minnesota Special Needs BasicCare (SNBC) Medicaid Only
UnitedHealthcare Community Plan of Minnesota is offering Special Needs BasicCare (SNBC) plan which serves people ages 18 - 64 who meet eligibility requirements beginning Jan. 1, 2023. You can choose your own doctor. And you get many extras that other plans don’t offer.
Minnesota Senior Care Plus (MSC+)
Minnesota Senior Care Plus (MSC+) is a managed care program that covers health care and Elderly Waiver services for people who are 65 and over and have Medical Assistance (MA). Our plan includes specialty and well-visits, care coordination, specific clinical trials, a range of dental coverage and more.

Our purpose is to really put community in the United Healthcare Community Plan.
And our commitment is to serve the most vulnerable.
Reach out, be present, impact lives, and that's really big for us. That's really some of the values that drive this Minnesota health plan.
We have connected in the last four months with over three dozen community based organizations.
We have connected in the last four months with over three dozen community based organizations.
And I am hopeful that we will have well over 100 events in any calendar year so that we truly do reach out and be present.
It shows that there's people out there supporting other people. They can rely on your neighbor, you can rely on the school, the Boys and Girls Club, UnitedHealthcare, whoever that happens to be.
Today we’re packaging about 2700 bags, 5400 meals.
People will be getting meals thanks to Meals on Wheels and our partnership with UnitedHealthcare.
It’s not just about funding those organizations to do the work. We’re participating in it.
And if we're going to address health inequities and racial disparities, we need to first have a very broad network and we were successful in doing that.
We're looking to make sure we have in our network providers that are not only within 30 minutes of a member, but within 30 minutes of a public transportation route of our members.
But a bigger part of what we do is really seeing our members as real people and that starts with us being in community and getting to know them.
We want to be here and we want to experience joy with you. We want experience happiness with you.
That's when we really start changing the dynamic. That's really when things start to shift.
