UnitedHealthcare Connected® for One Care (Medicare-Medicaid Plan)
Our UnitedHealthcare Connected® for One Care (Medicare-Medicaid) Plan is for adults ages 21 - 64 with disabilities who are eligible for both Medicare and MassHealth. The plan offers members a customized care plan and a single point of contact for all Medicare and MassHealth services.

Eligibility and Enrollment:
One Care
Our UnitedHealthcare Connected® for One Care (Medicare-Medicaid) Plan is for adults ages 21 - 64 with disabilities who are eligible for both Medicare and MassHealth.
You can choose to join UnitedHealthcare Connected® for One Care at any time of the year. To learn more about enrolling visit the Steps to Enroll page.
Senior Care Options (SCO)
- You must be 65 or older
- You also need to live in the service area
- Be eligible for MassHealth Standard only or MassHealth Standard and Medicare
- Must not have any other comprehensive health insurance, except Medicare
Benefits and Features
One Care MassHealth + Medicare offers:
- Dental Care
- Vision Care
- Transportation to and from some medical and some non-medical visits. This includes trips to and from the pharmacy to fill your prescriptions.

- Medical questions and situations come at inconvenient times. When you have questions about your health, you can call a nurse 24 hours a day, 7 days a week.
Senior Care Option plans offers:
You receive many benefits beyond those with traditional Medicare on a
UnitedHealthcare Dual Special Needs plan. That includes:
- A monthly credit to buy healthy food and OTC products or pay utility bills
- Copays as low as $0 for 3-month supply of generic and brand-name prescriptions with Optum® Home Delivery Pharmacy
- An allowance for covered types of preventive and comprehensive dental
- Vision, transportation and fitness benefits.
