UnitedHealthcare is working to deliver a simpler, more efficient experience for plan administration – and that includes easy access to the most frequently used documents to sell and manage UnitedHealthcare medical and specialty plans.
Earlier this year, plan administration forms, product grids, Prescription Drug Lists (PDLs), other administrative documents, benefit summaries and SBCs, and marketing materials for fully insured and level funded medical and specialty products were consolidated on the uhceservices.com website, making it a one-stop shop for plan administration, quoting new business, renewals and more.

After signing in to the portal using your One Healthcare ID, look for the Resources menu in the top navigation bar. In that menu, you’re one click away from:
Benefit summaries and SBCs: Digital benefit summaries and Summaries of Benefits and Coverage for medical and specialty plans
Forms: Applications and enrollment forms, installation/implementation forms and checklists, Prescription Drug Lists (PDLs), HSA forms, claims forms and more
Product grids and network documents: Plan/product grids, network brochures and fact sheets
Marketing materials: Overviews of UnitedHealthcare products and solutions, including behavioral health, pharmacy benefits, incentive programs, advocacy programs and more
Employee Engagement Planner: Member communications to help employees stay engaged with their health — and health plan — throughout the plan year

The addition of plan administration documents and forms for all states, funding types and group sizes to uhceservices.com provides a convenient, consistent experience across health plans and eliminates the need for state-specific websites.
Moving forward, uhceservices.com will serve as a single source of truth for documents at both the national and state level and is centrally managed by a single team to allow for more efficient management of documents and to ensure all forms and documents available are current.
As a result of this consolidation, other websites hosting forms and documents for single states/health plans will go offline Feb. 1, 2023.

Existing users of uhceservices.com for small group quoting and renewals can use their same One Healthcare ID to sign in.
